Referring Providers and Future Patients

Please have your referring provider send the referral by fax to (256) 517-8872.  The provider should clearly state the reason for referral and include the latest office note and pertinent lab results (e.g. urine culture results if you are being referred for recurrent urinary tract infections).

Faxes will be addressed in the order they are received.  

We apologize greatly for the wait to schedule and to have a first appointment.  We receive many referrals daily and have limited options for new patient appointments.  Please bear with us.  

Referrals are not always needed in order to be seen.  Call in appointment scheduling is allowed.  Same-day appointments are limited to emergency nurse visits for suspected urinary tract infections.

If your insurance requires an 'official' referral (e.g. Tricare, Medicaid, some BCBS plans, etc), it is your responsibility to have that referral in place by the date of your appointment.  If your required referral can not be verified, you will not be seen.
